As the parent / guardian of the child I am registering for this FITC session, I declare that all the information given is correct and that I understand that the school, organising authorities providing facilities, FITC and their employees are not under any liability whatsoever in respect of personal injury, loss or damage however caused unless criminal or negligent.

I also agree to allow FITC to collate and keep the data I have submitted until I request otherwise in writing (sent to the FITC head office address).
